Well Care Schedule 2017


We feel very strongly that vaccinating children on schedule with currently available vaccines is absolutely the right thing to do for all children and young adults. If you should absolutely refuse to vaccinate your child despite all our efforts, we will respect your decision but we will ask you to find another health care provider who shares your views.  

Newborn Recheck – usually around 4th or 5th day of life. Hepatitis B (if not given in the hospital). PKU if necessary – weight and color check.

1 Month – Weight, and growth check, Hepatitis B

2 Month – Pentacel (DTaP, IPV, Hib), Prevnar, RotaTeg

4 Month – Pentacel (DTaP, IPV, Hib), Prevnar, RotaTeg

6 Month – Pentacel (DTaP, IPV, Hib), Prevnar, RotaTeg

9 Month – Urinalysis, Hepatitis B, Developmental Screening (ASQ3)

12 Month – Prevnar, Hepatitis A, Chicken Pox, Hemoglobin, vision SPOT screen

15 Month – MMR and HIB

18 Month – Daptacel (DTaP) and Hepatitis A, Developmental Screening (ASQ3)

2 Year – Hemoglobin, Developmental Screening Tool (MCHAT), vision SPOT screen

3 Year – Vision SPOT screen

4 Year – Hearing, Vision – Cholesterol if needed

5 Year – Hearing, Vision, Proquad (MMR, and Varicella),  Quadracel (DTaP and IPV) – Required for Kindergarten entry

6, 7, 8, 9 and 11 Year – Hearing and Vision

10 Year – Hearing, Vision, and Cholesterol screening

12 Year – Hearing, Vision, Menactra and Tdap booster– Required for 7th grade, discuss adolescent vaccines

14, 15, 16, 17 and 18 Year – Hearing, Vision, possible Hemoglobin, possible urinalysis, possible lipid profile. Discuss adolescent
                                          vaccines.

*SHOTS FROM 2, 4, & 6 MONTHS OF AGE MUST BE GIVEN AT LEAST 6 WEEKS APART.

 *12-MONTH PHYSICAL MUST BE GIVEN NO EARLIER THAN 12 MONTHS OF AGE.

Well care is a necessary and mandatory part of care in our practice. 

Please be sure to know your insurance coverage before your visits.  Try to schedule your appointments early, so you can reserve a convenient time.  The schedule fills quickly during the summer, before school starts.  Shot records and Physical (HRS) forms are required for daycare entry, kindergarten entry, and seventh grade entry.  If these forms are needed please call in advance, or be sure to obtain these forms at your well care visits.
